Samuels Elementary School is partnering with the Immunization and Travel Clinic at Denver Health to offer in-school immunizations to students at Denver Public Schools in order to ensure students have a healthy school year. Next in-school immunization clinic date: November 9th
About the In-School Immunization Program:
One signed consent form allows your child to participate in any In-School Immunization Program clinic during the current school year.
If you have completed and returned a consent form, your child will receive any vaccines that they are eligible for on this date, including COVID-19 and flu, unless any specific vaccines were declined on the consent form. Please make sure the school nurse has your child’s most up-to-date immunization record.
Please make sure your child has eaten breakfast before coming to school on the clinic date.
If you wish to participate and HAVE NOT returned a signed consent form, please visit the nurses’ office to obtain one. Please turn-in your completed consent form to the school nurse by October 31st.
